In a truly connected community, help and resources are always within reach. This is why Summit County is partnering with Unite Us, a software company bringing sectors together to improve the health and well-being of communities. As the work grows, it will also help caregivers, policy leaders, and funders respond quickly to trends in social needs.

Summit County Opiate Abatement Advisory Council

Funding and support for this project come from the Summit County Opiate Abatement Advisory Council. The OAAC was formed to help Summit County Respond to the loss of life due to opioids in our community, Recover by expanding our capacity for treatment in Summit County, and Rebuild a new landscape for our community where there is no wrong door for someone to walk through to rewrite their story.

Unite Ohio is a coordinated care network consisting of healthcare, government, nonprofit, and other organizations. These network partners use Unite Us’ software to securely identify, deliver, and pay for services that address the needs of individuals within their communities. Learn more about the footprint in the state here 

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
